[3] A 2024 meta-analysis of 15 randomized controlled trials encompassing over 600 children with CP found that MSC therapy was associated with significant improvements in Gross Motor Function Measure (GMFM) scores compared to rehabilitation alone, with the strongest effects observed in younger children and those receiving multiple doses. Autism Spectrum Disorder (ASD)

Autism spectrum disorder affects approximately 1 in 36 children and is characterized by social communication deficits, restricted interests, and repetitive behaviors. Growing evidence implicates neuroinflammation — microglial activation, elevated cerebrospinal fluid cytokines, and altered blood-brain barrier permeability — in ASD pathophysiology. Hypoxic-Ischemic Encephalopathy (HIE)

Neonatal HIE occurs in approximately 1–3 per 1,000 term births and results from oxygen deprivation to the brain during the perinatal period. Despite therapeutic hypothermia — the current standard of care — roughly 30% of treated infants still die or survive with significant neurological impairment. [7] MSC therapy for HIE is being investigated as an adjunct to hypothermia, with preclinical studies demonstrating that MSCs reduce infarct volume, decrease microglial activation, increase neuronal survival, and improve long-term motor and cognitive outcomes. A pivotal Phase II trial reported that combining autologous umbilical cord blood MSCs with therapeutic hypothermia was safe and showed signals of improved neurodevelopmental outcomes at 1 year of age. Bronchopulmonary Dysplasia (BPD)

BPD is the most common chronic lung disease of prematurity, affecting up to 40% of extremely low birth weight infants. It results from arrested alveolar development and dysregulated inflammation in the immature lung exposed to mechanical ventilation and oxygen toxicity. [9] Multiple Phase I/II trials have demonstrated the safety of intratracheal MSC administration in preterm infants at risk for BPD, with secondary outcomes showing reduced ventilator days, decreased oxygen requirements, and lower rates of BPD severity. Additional pediatric conditions under investigation include spinal muscular atrophy (SMA), Duchenne muscular dystrophy, osteogenesis imperfecta, graft-versus-host disease (the only FDA-approved MSC indication, for pediatric steroid-refractory acute GvHD), and congenital heart disease. Each leverages a different aspect of MSC biology — immunomodulation for GvHD, trophic support for neuromuscular conditions, and osteogenic differentiation for bone fragility disorders.

Safety Considerations in Pediatric MSC Therapy

MSC therapy has an established safety profile in pediatric populations across multiple clinical trials. A 2023 systematic review of 55 pediatric MSC trials encompassing over 1,800 patients reported no cases of tumor formation, no ectopic tissue development, and no mortality attributable to MSC infusion. [11] The most common adverse events — transient fever and mild infusion reactions — occurred in fewer than 5% of administrations and resolved without intervention.

Growth and development. A unique consideration in pediatrics is that MSCs secrete growth factors — including VEGF, HGF, IGF-1, and FGF-2 — that theoretically could influence growth plate dynamics or organ development. Clinical Evidence — What the Data Show

The pediatric MSC evidence base, while still predominantly Phase I/II, is growing rapidly. Cerebral Palsy — Meta-Analysis Results

Limitations to Note: Most pediatric MSC trials remain early-phase with small sample sizes. Blinding is challenging when sham procedures are difficult to justify in children. هل MSC الحبل السري أفضل من MSC نخاع العظم للأطفال؟

تُفضل MSC المشتقة من الحبل السري في معظم بروتوكولات الأطفال لعدة أسباب: قدرة تكاثرية أعلى (مهمة لتصنيع الجرعات)، مناعة أقل (تقليل خطر الرفض بدون مطابقة HLA)، إفراز أقوى للعوامل الغذائية العصبية، ويتم حصادها من أنسجة الولادة المهملة بدون خطر على المتبرع. MSC نخاع العظم فعالة لكنها تتطلب حصادًا جائرًا وتظهر انخفاضًا في الفعالية مرتبطًا بالعمر.

كيف تبدو عملية العلاج للطفل؟

يبدأ العلاج بتقييم شامل يشمل مراجعة التاريخ الطبي والفحص العصبي وغالبًا التصوير الأساسي أو مقاييس التقييم المعيارية (GMFM للشلل الدماغي، CARS للتوحد). يستغرق الحقن نفسه 30–90 دقيقة ويتم تحت إشراف الطبيب مع مراقبة العلامات الحيوية. تشمل معظم البروتوكولات سلسلة من 2–4 حقن متباعدة 1–3 أشهر، مدمجة مع استمرار العلاج الطبيعي والمهني. تتابع تقييمات المتابعة في 3 و 6 و 12 شهرًا التقدم على مقاييس النتائج المعيارية.

هل علاج MSC معتمد للأطفال؟

باستثناء واحد — علاج MSC الخيفي لمرض الطعم ضد المضيف الحاد المقاوم للستيرويد عند الأطفال، الذي حصل على موافقة تنظيمية في اليابان (TEMCELL) وولايات قضائية أخرى — يبقى علاج MSC للحالات العصبية والتنفسية والمناعية لدى الأطفال استقصائيًا. إنه متاح في مراكز متخصصة مثل Velar Center كعلاج خارج التسمية مدعوم بالأدلة السريرية، لكن يجب على الآباء فهم الطبيعة الاستقصائية للعلاج وأن تكون لديهم توقعات واقعية حول النتائج.
